- Identify any challenges you might be facing, like not having a regular doctor
- Share gentle guidance if you’ve missed a wellness exam or might benefit from a check-in

What’s Inside
You’ll answer a few simple questions about:
- Your health history and any current conditions
- How you’ve been feeling
- Whether you’d like help from a direct primary care provider
We’ll also ask if you’d like us to securely share your answers with a provider associated with Mending to better coordinate support.
