A Thank You for Taking Care of Yourself
At Mending, we believe prevention is powerful. When you take time for an annual wellness visit, you're investing in your health, and we want to celebrate that.
That's why we created the Wellness Visit Reward, a simple program that thanks you for completing your yearly checkup.
How It Works
The program is straightforward:
- Schedule your visit. Book an annual wellness exam with any primary care provider in our network, including Direct Primary Care doctors or traditional practices.
- Complete your appointment. Show up, get checked out, and have a conversation about your health.
- Upload proof of your visit. After your visit, fill out our short form here.
- Receive your reward. Within 60 days of your visit and verifying your submission, we'll email you a $50 Visa gift card as a thank you.
Why We Offer This
Wellness visits matter. They help catch health concerns early, keep you up to date on screenings, and give you a chance to talk openly with your doctor about how you're really doing.
What You Need to Know
- Available to all members enrolled in our ACA marketplace plans
- One reward per member, per plan year
- Your wellness visit is already covered at no cost to you under preventive care benefits
- The gift card is in addition to your covered visit, not a replacement
- The wellness visit must be completed between 11/01/2025 and 12/31/2025 to qualify for the reward
When You'll Get It
Once your provider submits the claim or encounter data confirming your visit, our team will verify your form submission and eligibility, and then process your gift card. You should receive it by email within 60 days after verification of your visit.
